The study included 750 consecutive patients with acute stroke or transient ischemic attack. During hospital stay as well as three months, twelve months and five years after stroke a full neuropsychological examination was performed. Patients who failed to attend the appointments were interviewed by telephone. The results of this study revealed that dementia is a common complication following stroke, however there are no unambiguously efficacious methods of prevention and management. Patients after stroke with delirium during hospital stay are more likely to develop dementia in the short term after stroke but the effect of delirium on dementia seems to decrease with time. Transient cognitive impairment affects over 50% of patients in the acute phase of stroke, but does not increase the risk of long-term complications. Patients with in-hospital delirium more often develop transient cognitive impairment. Considering long-term follow-up, more than half of patients die within five years after stroke. Post-stroke delirium and post-stroke dementia negatively affect five-year prognosis increasing risk of death and disability.
