Airway remodeling in asthma contributes to the persistence of lung functional impairments and the development of severe asthma phenotype. Biomarkers of airway remodeling and effective therapies targeting this process are currently unavailable. The study aimed to correlate tomographic features of airway remodeling in n=105 patients (including 53 with fixed airway obstruction) with the clinical characteristics of asthma patients, laboratory test results including cytokine levels in serum and bronchoalveolar lavage, pulmonary function test results, and bronchial epithelial basement membrane thickness in search of factors associated with airway remodeling. By identifying eosinophilic infiltration in the airways as a factor correlating with remodeling features, gene transcriptomes of bronchial brushings from n=40 patients (including 20 with eosinophilic asthma) were compared to identify sets of genes undergoing differential expression and co-expression. To search for biomarkers and potential therapeutic targets, the expression of the aforementioned genes was correlated with tomographic features of remodeling, pulmonary function test results, and enrichment analysis of ontological concepts was conducted to determine signaling pathways potentially related to airway remodeling, including TGF-p/Smad2/3, E2F/Rb, and Wnt/0- catenin.
